Disciplinary Records(Student must be in good standing at time of registration)

Social Security Card

Birth Certificate

Tennessee Immunization Form

(2) Proofs of Residency (utility bill, realtor contract, apartment contract)

Student MUST be accompanied by a Parent/Guardian
